Offline Forms Packet

We make available an easy to follow Account Information Forms Packet that you may print out. This packet provides forms with ample empty space for all the information to be stored in a MedsFile.com account.

The patient can fill out this form by hand, and then give the packet to a designated computer-savvy caregiver or loved one who can join our service for the patient and act as the administrator for the account.

Once the patient's account is established, the account administrator can give the patient a customer ID card. With this, the patient never has to remember any medication or medical information. The next time the patient goes into the doctor's office, and the nurse asks, "What medications do you take at what dosages?" the patient doesn't have to remember. All the patient has to do is hand the customer ID card to the nurse. The nurse can access and print the file.
